Please see below the healthy lifestyle programmes that OneLife Suffolk provide. For each service, we have created a client pathway to support you when making referrals to us. These include information about our services, patient benefits, eligibility criteria, and how to refer to.

Get Help To Get Active

Sorry. You are now no longer able to refer into the Get Help to Get Active service.

Adult Weight Management

You can refer clients/ patients to the Adult Weight Management support through the OneLife Suffolk service until Friday 29th September. OneLife Suffolk groups have now ended, however clients/ patients will be issued a 12 – week Slimming World voucher which will offer them support until December 2023.

NHS Health Checks

Will continue to be offered up until Friday 29th September on an ‘as and when’ basis.

The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia.

Health Walks

OneLife Suffolk offer over 200 free, volunteer-led Health Walks across all of Suffolk. All walks are graded from 1-5 based on their difficultly, ensuring there is a walk suitable for everyone.

Stop Smoking

The final date for new stop smoking referrals will be Friday 18th August.   

We apologise if you received communications that suggested an earlier cut-off date for new referrals. We can confirm that this was sent in error, and you still have until the end of this week. This deadline for referrals allows the OneLife Suffolk team the time to triage new patients, offer an initial consultation, provide them with access to treatment and support them to achieve a four-week quit. 

OneLife Suffolk’s Stop Smoking service provides supportive behaviour change therapy to help patients quit smoking for good. With OneLife Suffolk’s support, patients are three times more likely to quit smoking for good.

Families and Young People Service

OneLife Suffolk School programme

The OneLife Suffolk Families and Young People service runs a 10 week family programme that supports families to lead healthier lifestyles and form sustainable positive habits that the whole family can benefit from.

Tier 1:

  • Level 1 services are available for all children, young people and parents living in Suffolk. There is no access criteria for this service; our free webinars can be found here.

Tier 2:

  • Children between 5 – 18 years who have a BMI > 91st centile on the growth chart
  • The child, young person and family must live in Suffolk

Refer into our Tier 2 children’s service by completing our online referral form.

Tier 3 Multi-disciplinary Weight Management service:

  • Children between 5 – 18 years who have a BMI > 91st centile with comorbidities and require physiological, psychological, social, educational or emotional support.
  • Children between 5 – 18 years who have a BMI > 95th centile without comorbidities or complex needs.
  • Geographical criteria also apply due to commissioning criteria.
