Children’s Healthy Lifestyle Practitioner

 

Responsible for the delivery of the children’s services; which include family clubs, holiday clubs and school programmes.

  • Following the curriculum, plan the sessions to be interactive and engaging, considering the needs of the clients in the group
  • Facilitate lifestyle & activity sessions that are developmentally appropriate and relevant for all involved

The initial pilot has been funded for a year with the potential to secure further funding going forwards.

OneLife’s Tier 3 service for children will adopt a family-based and person-centred approach to support children who are above a healthy weight range and who also face complexities such as, physiological, psychological, social, educational or emotional difficulties. The programme will be delivered through an initial 14-week intensive phase, followed by bi-weekly and then monthly sessions to offer ongoing support for up to 12 months.

Dietetic Lead (Fixed Term for 1 Year).

Due to the expansion of OneLife Suffolk, we need to recruit to several new posts so that we can continue delivering amazing services to Suffolk residents – after all, we are here to serve our local community.

We are looking to appoint a Dietetic Lead to join the team at this exciting time.

This is a full-on role – you will be out in the community overseeing and delivering our amazing Tier 3 Children’s Weight Management programme to families from your local community who want to make healthier lifestyle choices.

No two days are ever the same – as everyone that you work with will be different!  The young people and their families that we work with have many reasons for wishing to manage their weight – it may be a long-term health condition, a desire to feel fitter and be more active – or to be able to ‘do more’ with their friends and family.  The best news is that you will be an integral part of their journey – and that is a satisfying feeling!

You’ll need to be a confident group leader – full of energy, engaging, great with children – able to talk to real people about their lives.  What’s even better is that our programmes aren’t just about the ‘here and now’ – you will be supporting the delivery of the service over the long-term – following up clients for a year after their group finishes – helping  children and their families to maintain healthy lifestyle changes and providing some extra support when needed.

Psychology Lead

The Psychology Lead will be responsible for the clinical supervision of the delivery teams. They will ensure that the programmes are delivered to the highest standard to achieve positive outcomes for the individuals involved, in a cost-effective manner. In conjunction with the Contract Manager, the Clinical Lead will be the key point of contact with the Commissioners and Corporate Customers and lead the on-going development of the service.

The role will need to provide clinical guidance as part of an MDT regarding patients who have been referred to the service who have psychological needs.  The role will involve identification of an optimal care pathway for these individuals. As part of our team, you will require sound in-depth knowledge of obesity prevention and treatment, dietetics, health psychology and public health.

The role will involve joint working across the team to support the ongoing development of curriculums and content (articles, blogs, vlogs, podcasts) that supports and engages services users, it will also.  This will be across a variety of themes from well-being to behaviours change and across our lifestyle services.  Content will also be developed to support colleagues and wider stakeholders.

The role will engage with research teams to enable a continual dynamic process of implementation of evidence-based practice and practice-based evidence.

Outreach NHS Health Check Practitioner (Maternity cover) 

An NHS Health Check is a screening for 40 – 74 year olds which assess an individual’s cardiovascular risk and aims to identify any early signs of long term conditions such as Stroke, Type 2 diabetes and Heart Disease. The NHS Health Check Practitioner is responsible for taking measurements, including cholesterol, blood pressure and weight. If you have a passion for supporting local people to live healthier lives, we are recruiting for an NHS Health Check Practitioner where full training will be provided. This role will be predominately for the Ipswich and East area of Suffolk although the flexibility to cover work in the West Suffolk and Waveney area may be required.    

As an NHS Health Check practitioner, other responsibilities will include:

  •  Engaging with a range of Voluntary, Community, & Faith sector (VCF) and Health & Social Care Professionals, as well as communities and localities, to identify groups and clients who may benefit from the NHS Health Check but who may not traditionally engage with health promotion activities or NHS services.
  • Work with these groups and the Health Check Team to deliver the NHS Health Check to identify people at risk of cardiovascular disease in communities which have identified health inequalities.
  • Provide agreed health improvement advice to all clients identify those who will benefit from signposting to health lifestyle interventions via the OneLife service, as well as the client’s General Practitioner as per agreed protocols.
  • Improve the uptake of the OneLife services via the NHS Health Check route.
  •  Promote the equality and value the diversity of individuals.
