Outreach Health Check Practitioners

One part time role working across Ipswich & East (although travel throughout the county is essential) 16-18 hours per week part-time

One Full time role based in Waveney (although travel throughout the county is essential) 37.5hrs per week.

Salaried at £18,000 – £22,000 pa (prorata) dependent on experience, plus 1% pension contribution rising to 3% by April 2019

Reporting to Health Checks Lead/Clinical Lead for Health Checks

Key Relationships – Managers / Stakeholder Link Practitioners / Referral Generation Officer

MAIN DUTIES AND RESPONSIBILITIES:

  • Engage with a range of Voluntary, Community, & Faith sector (VCF) and Health & Social Care Professionals, as well as communities and localities, to identify groups and clients who may benefit from the NHS Health Check but who may not traditionally engage with health promotion activities or NHS services.
  • Work with these groups and the Health Check Team to deliver the NHS Health Check to identify people at risk of cardiovascular disease in communities which have identified health inequalities.
  • Provide agreed health improvement advice to all clients identify those who will benefit from signposting to health lifestyle interventions via the OneLife service, as well as the client’s General Practitioner as per agreed protocols.
  • Improve uptake of the OneLife services via the NHS Health Check route.
  • Promote the equality and value the diversity of individuals.

Health Walks Practitioner

Covering West Suffolk (Travel within Suffolk will be required), 18.75 hours a week

Salaried at £18,000 – £22,000 pa (prorata) dependent on experience, plus 1% pension contribution rising to 3% by April 2019

Reporting To: Health Walks Lead

Key Relationships: Managers / Delivery team /Client services team

Job purpose:

The health walks practitioner will plan and deliver health walks programme including the recruitment, induction and support of volunteer walk leaders, and day-to-day management of the service. The role will ensure the effective planning and promotion of the walks’ programme including compliance with Walking for Health requirements. The post-holder will maintain a register of volunteers and ensure new volunteers undergo an induction programme. This role would also include the co-ordination of the Health Champions project to help combine the volunteer section of the business.  The practitioner will work closely with marketing staff and will represent the service externally to potential volunteers and partners including: parish and district councils, voluntary organisations, the media, and regional and national walking bodies.

Programme Lead Health Walks

18.75 hpw, flexible hours including evenings and weekends.

Salaried at £25,000 pro rata * plus 1% pension contribution rising to 3% by April 2019.

Place of work; Ipswich (Travel within Suffolk will be required.)

You will report to the Adult Services Manager and have key relationships with Managers / Delivery staff / Psychology Lead / Dietetics Lead / Client Services Team.

You will work with the Adult Services Manager and overall OneLife and MoreLife management teams to:

  • Deliver the KPIs of the relevant service, and support the overall delivery of the OneLife KPIs
  • Co-ordinate the organisation and delivery of the relevant service, including staff allocation, venues, resources etc.
  • Lead and manage the team and provide feedback on the service, delivery and curriculum, as well as to line managed staff to support an effective HR strategy: recruitment, training, induction, CPD, performance management and 1:1s, following all policies and procedures
  • Effectively govern the service – clinically and operationally (IG, safeguarding, risk, health and safety etc.)
  • Promote the service and create referrals, creating links with a range of partners including voluntary organisations and community groups
  • Provide delivery assistance, if required
  • Liaise closely with the clinical lead.

The programme lead will ensure the service area is delivered according to the terms of the contract across Suffolk and will achieve the KPIs within the defined budget.

Health Checks Referral Generation Officer

Approximately 20 hpw Monday to Friday in Ipswich, working flexibly to meet service needs.

£18,000 pro rata* 1% company pension contribution rising to 3% by April 2019. Potential bonus depending on performance.

You will be responsible to the Health Checks Lead and have key relationships with Health Checks Lead / Marketing and Campaigns Lead / Delivery staff / Administrative staff / Key stakeholders.

Job purpose:

  • To maximise the effectiveness of the Health Check Practitioners by producing high quality leads, working as a team towards agreed set targets
  • To provide a high level of referral general leads by ensuring all queries are responded to in an efficient, professional and timely manner
  • To follow up Health check leads and generate health check appointments.

Stakeholder Link Practitioner

£18,000 – £22,000* plus 1% pension contribution rising to 3% by October 2018.

37.5 hpw based in East Suffolk, with travel in the area essential.

You will report to the Programme Manager Engagement Team and have key relationships with the Programme Managers / Leads / Stakeholder Link Practitioners.

You will contribute to the Engagement Teams aims and objectives, supporting the OneLife Suffolk offer to:

  • Increase awareness and engagement across Suffolk through locality link working
  • Understand your role and responsibility to ensure all KPIs are achieved including MECC training, Public Health Campaigns and events
  • Ensure all activities, events and training are effective and contribute to the OneLife aims, objectives and KPIs.

Children activity & lifestyle leaders for clubs in Suffolk

MoreLife is looking for Lifestyle Leaders for a number of contracts. Lifestyle Leaders are expected to prepare and run lifestyles sessions in line with the clubs curriculum to parents and children, and to assist in the organisation and delivery of all group activities. This will lead to the provision of a safe play and educationally stimulating, fun environment for all individuals.
